- Application for Individuals: Form 12A, copy of ID, copy of mineral tenement, location plan. - Application for Companies: Form 12A, copy of Memorandum of Articles, various company forms, copy of mineral tenement, location plan. - Application for Cooperatives/Corporations/Organizations: Form 12A, Articles of Association, copy of mineral tenement, location plan. - All documents must be submitted in sets of four.

Application Fee: · Application form RM 1.00 / piece (4 pieces) · Mining Application RM 50.00/application Approval Fee: · Licence Preparation RM 100.00 · Licence Issuance RM 100.00 Validity Period: · Cannot exceed the expiry of the mineral tenement for which water is to be used.
