- For new applications, individuals must provide 2 copies of their identity card and spouse's identity card (if applicable). - For companies, 2 copies of company registration (SSM), M&A, Form 24, Form 44, and Form 49 are required. - For associations/organizations, 2 copies of registration, constitution, and meeting minutes approving the application are needed. - PTNS-2/96-3 form and area plan must be submitted.
