- Application letter addressed to the District Officer of Kuala Muda - 6 copies of the Entertainment Licence application form (BLH 01) submitted at least three months before the intended operation date or before the current licence expires - 6 copies of the front/back of the identity card (A4) - 4 passport-sized photos of the applicant (in color) - 6 copies of details of the performance/show and participants - 5 copies of the support letter from the Central Agency for Film Shooting and Foreign Artist Performances (PUSPAL) if involving foreign artists - 6 copies of the location plan of the performance - 6 copies of the landowner's permission letter - 6 copies of land grant/tax (for review by the Land Management Division, Kuala Muda District Office)
