Application requirements
- The applicant must complete the Entertainment License Application Form under the Entertainment and Places of Entertainment Enactment 1998.
- Required documents include:
a) 1 Application Letter
b) 4 copies of the applicant's identity card
c) 4 copies of the organization registration
d) 4 copies of the committee members
e) 4 copies of the location plan
f) 4 copies of the program schedule
g) 4 copies of land ownership/land lease agreement
h) Any other necessary documents as deemed required from time to time.

1. Chinese Prayer - RM 20.00 per day
2. Chinese Opera - RM 20.00 per show
3. Banquet - RM 20.00 per day
