Application requirements

- Voluntary for pet owners. - Mandatory for pet owners traveling abroad. - Application at Veterinary Service Office or selected private veterinary clinics. - Pets must undergo physical examination, vaccination, and microchip installation. - Complete one information form for the pet. - All owner and pet information must be registered in the Pet Passport system, Department of Veterinary Services (DVS) Malaysia.

ONE-OFF FEE 1) Physical examination: RM 10.00 per pet 2) Microchip installation: based on current supply price 3) Pet Passport book: Cat, Dog, Rabbit: RM 7.00 per pet Horse: RM 9.00 per pet **The Pet Passport book is valid for one animal and lasts for the animal's lifetime.
