- Composite Business Licence Application Form (RM 3.00)
- Copy of applicant's Identity Card
- One passport-sized photo
- Copy of the latest TY2 Injection Certificate for the applicant and workers (for food and beverage businesses)
- Copy of Food Handling Course Certificate for the applicant and workers (for food and beverage businesses)
- Copy of Business Registration Certificate (SSM)
- 2 copies of land ownership (if applicable)
- Letter of permission from landowner if operating on private or government land (if applicable)
- Business location photo
- RENEWAL OF LICENCE
- Current licence
- Latest SSM
- Copy of the latest TY2 Injection Certificate for the applicant and workers (for food and beverage businesses)
