- One (1) copy of company registration (ROC) / business (ROB) / organization (Form 9 and Form 49 or Form D with business and owner information) - One (1) copy of identity card OR passport - One (1) copy of certificate of occupancy (CF) OR Certificate of Compliance (CCC/CFO) - Floor plan and photos of premises (exterior, interior, and rear) - Approval letter for premises modification (if any modifications are made inside or outside the premises) - This licence will be considered void if payment is not made within six (6) months from the date of this approval letter.
