- Official application letter to the District Officer of Kubang Pasu.
- 3 copies of Form 'Schedule One Rule 3 of the Letter Writers Registration and Licensing Rules 1990.
- Original support letter from the State Assembly Member (ADUN) for new applications.
- 3 copies of the applicant's identity card.
- 3 photographs of the applicant.
- Copies of educational certificates and other relevant documents.
