Application requirements
- One (1) copy of the applicant's identity card.
- One (1) copy of the pyrotechnician's identity card.
- One (1) copy of the Pyrotechnic Expert Certificate.
- One (1) copy of the Insurance Certificate for the event.
- One (1) copy of the Company Registration Certificate (Form 9).
- One (1) copy of the Company's Board of Directors (Form 49).
- One (1) copy of the End User Letter (if applicable).
- One (1) copy of the Fire and Rescue Department Report.
- One (1) copy of the Department of Civil Aviation (DCA) Report – if required.
- One (1) copy of the Local Council Report.
- One (1) copy of the Site Owner's Permission Letter.
- One (1) copy of the Organizer's Offer Letter.
- One (1) copy of the Home Ministry Approval Letter.

RM1.00 (Pol. 124) - importing, exporting or transferring explosives
Validity period - only valid as per the requested date (ONE-OFF)
