Application requirements

- Official company application letter. - Application form (Form BKK-PKK 1). - Copy of distributor appointment letter (if not the manufacturer) - foreign appointment letters must be certified by a local commissioner for oath. - Copy of the latest test report - any foreign test report must be certified by a local commissioner for oath. - Company profile (such as organizational chart, workflow, etc.). - Brochure and supporting documents. - Mini sample of material/system (with company label, material name). - For fire suppression system (halon alternative) applications, additional documents are required.
