- Include a copy of company documents certified by a Commissioner for Oaths/Company Secretary as follows:
- For Sole Proprietorship/Partnership: 1 copy of Business Registration Certificate (Form 'D') for Peninsular Malaysia.
- 1 copy of Business Registration Certificate (Form 'B') for Sabah.
- 1 copy of Business Registration Certificate (Form 'I') for Sarawak.
- 1 copy of FINAS License.
- 1 copy of FINAS License Exemption Letter (if applicable).
- For incorporated companies (Sdn Bhd/Bhd): 1 copy of Company Registration Certificate (Form 9 or Form 13).
- 1 copy of Memorandum and Articles of Association.
- 1 copy of Form 24 and Form 49 (Companies Act 1965).
- 1 copy of FINAS License.
- 1 copy of FINAS License Exemption Letter (if applicable).
- For other institutions (associations/unions/cooperatives): 1 copy of Business Registration Certificate.
- 1 copy of Constitution/By-laws.
- 1 copy of the Committee Member list.
