- Completed film censorship application form - Statutory Declaration (Stamped by Commissioner for Oaths) - Company ownership letter (copyright) - Film synopsis - Attachments containing details of Language, Format, Subtitles, Actor Names, and Production Company - DCP/35MM/DVD - Two (2) copies of completed application form for tape - VHS/CD/DVD/etc. - Cover/Front Cover/Inlay (Original Colored Front Cover and Copy of Front Cover) - Synopsis - Attachments containing details of Language, Format, Subtitles, Actor Names, and Production Company - Two (2) copies of completed application form for advertisement - Two (2) colored storyboards - CD/DVD/USB drive - FINAS certificate for Malaysian/Imported production - Supporting documents (Halal certificate/Notification Note/related documents)
