- Three (3) passport-sized photos
- One (1) copy of Identity Card
- One (1) copy of Memorandum & Articles of Association
- One (1) copy of Company Commission of Malaysia (SSM) certificate
- One (1) copy of Company Directors' details (Form 49)
- One (1) copy of Attachment 'A' to Pol. 122A
- One (1) copy of land grant for magazine construction (if applicable)
- One (1) copy of authorization letter for land excavation
- One (1) copy of site plan showing magazine location
- One (1) copy of magazine drawings certified by a registered architect/consultant
- One (1) copy of approval letter from Local Authority for magazine construction
- One (1) copy of approval letter from landowner for magazine construction
Agency fee breakdown
1. New Application/Renewal
1.1 RM200.00 per year (storage of explosives in bulk or floating magazine)
1.2 RM25.00 per year (storage of explosives in a magazine or store exceeding 18 cubic meters)
1.3 RM15.00 per year (storage of explosives in a magazine or store not exceeding 18 cubic meters)
1.4 RM10.00 per year (storage in a shop dealing with explosives for a maximum stock of 3,000 percussion caps and 9 kg of explosive powder)
1.5 RM100.00 (shop dealing with explosives exceeding the above limits)
1.6 RM10.00 per year (storage in a shop dealing with fireworks only)
1.7 RM1.00 (temporary storage of explosives)
1.8 RM1.00 (duplicate licence for lost/destroyed licence)
2. Name Change Application
Fee - RM25.00
3. Quantity or Type Change Application
Fee - RM25.00
