- Composite Form (DBKL-JPL-KP-01-A) for Application of Premises and Signboard Licence (this form is free).
- Copy of Company/Business/Organization Registration (Form 9 & 49 or Form D along with business and owner information).
- 1 copy of the applicant's identity card/passport.
- 1 colored visual of the signboard advertisement and a picture of the installation location (if applicable).
- 2 exterior photos of the premises from different angles (including adjacent premises, above and below) and 2 interior photos from different angles.
- Advertisement visuals certified by Dewan Bahasa & Pustaka.
