- Copy of the original grant (extraction area and land placement area)
- Plan of the lot area applied (extraction area and land placement area)
- Letter of consent from the landowner (if necessary) if passing over someone else's land.
- Copy of the applicant's Identity Card
- Copy of MAA / Societies Act if registering under the company's name.

6. Coral, Shell - RM 10.00 per cubic metre
7. Shale - RM 2.00 per cubic metre
8. Gravel - RM 6.00 per cubic metre
9. Debris - RM 1.00 per cubic metre
10. Lime - RM 2.00 per tonne
11. Limestone - RM 3.00 per tonne
12. Turf - RM 1.00 per cubic metre
13. Guano - RM 15.00 per tonne
14. Rock, stone or granite in any form - RM 3.00 per tonne
15. Quarry waste - RM 3.00 per cubic metre
16. Clay bricks & concrete bricks - RM 1.00 per 100 pieces
17. Tiles - RM 1.00 per 100 pieces
18. Marbles - RM 10.00 per one tenth of one cubic meter
19. Precious stone - RM 2.00 per 100 cubic centimeters
