- For new applications, individuals need copies of their identity card and spouse's identity card (if applicable).
- For companies, copies of company registration (SSM), M&A, Form 24, Form 44, and Form 49 are required.
- For associations/organizations, copies of registration, constitution, and meeting minutes approving the application are needed.
- Application form PTNS-2/96-3 and area plan must be submitted.
- Application fee and plan (RM28.00) are required.
- For renewal applications, the renewal application form and the latest corporate information from SSM are needed.
