Permit for Excavation Work in JKR Road Reserves (PEW)

- Two (2) copies of the location plan including key plan and detailed plan showing excavation depth, proposed pipe/cable size, and distance from road reserve edge. - Two (2) copies of the method of statement and complete specifications for excavation, backfilling, and road restoration. - Two (2) copies of the method statement for jacking (Thrustboring, Horizontal directional Drilling, etc.). - Two (2) copies of the river crossing plan showing the position at the end of the road reserve at the involved bridge location. - Two (2) copies of detailed pipe support plans showing positions near high land and slopes. - Two (2) copies of the proposed schedule of work according to chainage. - Two (2) copies of plans showing typical worksite location of barricade, excavator/backhoe, and movement of plant within work area. - Two (2) copies of traffic plans detailing safety signage layout. - Four (4) completed and signed permit application forms. - Signing of the agreement form.

25% - State revenue 75% will be refunded after the defect period of one (1) year after the Certificate of Completion is issued.
