- Individual (2 copies): Identity Card of Applicant, Identity Card of Spouse (if any) - Company (2 copies): Company Registration (SSM), M&A, Form 24, Form 44, Form 49, Copy of Identity Card of Director (one) - Association/Organization: Copy of Registration, Constitution/By-Laws, Meeting Minutes approving the application - Form 4: Application for temporary occupation license/permit - Form 8 (2): Application for permit to extract, transport, and move rock materials (from government land/reserved land/mining land) - Export Form: If rock materials are to be taken out of the district - Area Plan: Each application plan must be signed

APPLICATION FEES - Application Form Fee (Form 4) - RM2.00 (2 copies) - Application Form Fee [Form 8(2)] - RM2.00 (2 copies) - Plan Fee - RM20.00 - Registration Fee - RM20.00 POST-APPROVAL FEES - Royalty Fee: RM3.00 per cubic meter - Annual Fee: RM 3,500.00 per hectare - Deposit Fee: 15% of the total value of rock materials to be extracted as per KTP.39(2)(VI) - Document Fee: RM15.00 per book (+ max. 50 sheets) VALIDITY PERIOD - Valid until December 31 of the current year.
