- Official application letter to the District Officer of Kubang Pasu - Applicant must not be Muslim - 5 copies of Excise Form 27-Pin1/84 - 5 copies of the applicant's identity card - 5 passport-sized photographs - 5 copies of the Business Registration Certificate (SSM) - 5 copies of the approved location plan and building floor plan - 5 copies of Business Licence Forms 49 and 9, if a private limited company - 5 copies of newspaper advertisements (in Malay & English) for new applications.
