- New Application/Renewal - Must pass theoretical and practical blasting examinations. - Submit application to the JMG State Office along with the following documents: - 1. Completed JMG Form (LK/OPS) KP1 - 2. Copy of Blaster's Certificate - 3. Copy of Identity Card - 4. 2 passport-sized photographs. - 5. Copy of Practical Blaster Approval Letter (new application only)
