- Copy of MyKad of the applicant or responsible officer. - Copy of Business Registration from the Companies Commission of Malaysia (SSM) or Cooperative Registration from the Malaysian Cooperative Commission (SKM). - Copy of business licence from the Local Authority (PBT). - Proof of legal ownership of land and premises/lease/rental/lease purchase agreement/occupancy permission from the owner. - Bank statement or document to prove minimum capital of RM50,000. - Compliance with the location requirements for premises.
