- 1 copy of the applicant's identity card - Completed application form for the extraction and collection of rock materials - 1 copy of the ESCP land plan (topographic plan) by a licensed surveyor - Copy of the land revenue receipt for the current year - Letter of consent from the landowner if the applicant is not the landowner - Letter of consent from the landowner if using other land as access - Photographs of the land condition if available
