- a) 4 copies of the application letter
- b) 4 copies of the applicant's identity card
- c) 4 copies of the organization's registration

- e) 4 copies of the location plan
- f) 4 copies of the program itinerary
- g) 4 copies of land ownership or land lease agreement
- h) Any other documents deemed necessary from time to time
