Permanent Entertainment Licence (LHT)

- New entertainment licence applications are frozen. - Renewal applications will be processed if the application requirements are complete. - Documents required for snooker licence renewal include a letter of application addressed to the District Officer of Kuala Muda, six copies of the entertainment licence application form (BLH 01), six copies of the front and back of the identity card, four passport-sized photos of the applicant, six copies of the Business Registration Certificate (SSM), six copies of the sale or rental agreement of the building (with a sworn declaration from the shop owner), six copies of the location and floor plan of the building, six copies of colored photos of the shop's exterior and interior, six copies of the land grant/tax (for comments from the Kuala Muda District Land Office), six copies of the MPSPK business premises licence, and six copies of the latest entertainment licence for renewal.

Service Charge: RM10.00 Licence Fee: RM2000.00 (per table) The validity period of the licence is 1 year or according to the validity period approved by the Licensing Committee in the Kuala Muda District Licensing Committee meeting.
