- Copy of Business Registration Form (Form 9/SSM)
- Municipal Council business licence from Jasin / Hang Tuah Jaya

- List of committee members/organization/board of directors (Forms 24 & 49)
- Rough plan of premises & location plan for business operation
- Newspaper advertisement in two consecutive days in two languages (Malay newspaper is mandatory)
