- Application for renewal, transfer, or new liquor selling licence
- 5 copies of the application form
- 5 copies of the advertisement in 2 newspapers (new application)
- 5 copies of the applicant's identity card
- 5 copies of the existing liquor licence
- 5 copies of the business licence MPS/MPN
- 5 copies of the business registration – Companies Commission of Malaysia (SSM)
- 5 copies of Form 49 – Companies Act, 1965 (new application)
- 5 copies of the licensed premises location plan
- 2 photos of the exterior of the premises showing the business name sign without images and 2 photos of the interior showing the liquor storage area.

Public House Licence Class I Annual Fee = RM 2,640.00
Public House Licence Class II Annual Fee = RM 1,920.00
Public House Licence Class III Annual Fee = RM 1,320.00
