- One (1) set of application form (Form A, Regulation 3) with a cover letter - Safety Data Sheet (SDS) - Research Protocol - Support letter from other Government Departments/Universities/other Government Agencies (if any) - Letter of permission from the farmer/owner to use the crop area as a trial plot - Fee: RM10.00
