Application requirements
- List of committee members (full name, old and new ID numbers, home address, date of birth, age, and position)
- List of collectors (aged 16 and above) (full name, old and new ID numbers, date of birth, age, and home address)
- Certificate of Registration of Societies (Form 9) for associations/temples
- Sample donation card, jogathon, walkathon, official receipt (must match the name of the organization)
- Sample collector badge (badge must match the name of the organization)
- Support letter from the relevant ministry (for applications from organizations or educational institutions)
- Minutes of the organization's meeting regarding approval to collect donations
- Copy of the organization's constitution (stating that the organization can collect donations for its fund)
- Letter from the Registrar of Societies confirming the organization is active and its constitution allows for donation collection.
