- One (1) copy of company registration (ROC) / business registration (ROB) / organization (Form 9 and Form 49 or Form D with business and owner information)
- One (1) copy of identity card OR passport
- One (1) copy of certificate of occupancy (CF) OR Certificate of Compliance (CCC/CFO)
- Floor plan and photos of the premises (exterior, interior, and back)
- Approval letter for premises modification (if any modifications inside or outside the premises)
- Certificate/letter of medical examination including Typhim VI vaccination and Food Handler Course certificate for all food handlers and workers (for food vendors only)
- For renewal application, a copy of the licence to be renewed
