Application requirements
- Application form for registration of residential/day care center.
- Copy of the operator's identity card/passport.
- Copy of Business/Company Registration Certificate (SSM) or Organization Registration Certificate (ROS) or Parliamentary Act or Ordinance.
- Copy of approval letter from Technical Agencies: (a) Local Authority (PBT) (b) Fire and Rescue Department (c) Health Department (d) Land and Survey Department (Sarawak only).
- Copy of the floor plan of the care center premises and calculation (square meters).
- Ratio of caregivers to residents.
- Daily activity schedule.
- Menu schedule for residents.
- List or schedule of activities or involvement of parents/guardians/heirs.
- List of names of residents/persons taken for care.
- List of names of caregivers/staff along with copies of identity cards/passports.
- Location plan of the care center premises.
- Copy of health inspection/vaccination recommended by the Health Department.
- Copy of certificate/other related documents.

Application for Care Center Registration
Registration fee for care center is RM50 for a period of five years.
