- Application letter addressed to: District Officer Kuala Muda, District and Land Office Kuala Muda 08000 Sungai Petani Kedah Darul Aman.
- 6 copies of identity card (A4) front/back.
- 2 passport-sized photographs of the applicant (in color).
- Applicant's experience information (resume) and educational background.
- Written permission to use the office space/building issued by the building owner or management agency.

Licence Fee: RM20.00 (Annually)
Validity period of the licence is 1 year.
